Jsa & H/B+Applicable amount

Hi, In mid Sept 2009 my JSA stopped along with my H/B & CT/B & after filling in a 'Review' form from the LA I then received H/B etc through my applicable amount. In Feb 2010 I was in receipt of JSA again so my H/B etc entitlement came by way of my passported benefit. My question is would I have had to fill in another application form for my H/B whether it be a Rapid Reclaim (HBRR1) or a fresh claim HBCTB1 please?
