Vulnerable Persons and the Police

Can anyone help with this, please? When someone signs a "Vulnerable Person Form" with the Police, the person's details are forwarded to what they call, "other agencies". Does anyone know if the Police have to tell the person who all these other agencies are and can the person have this form and their details deleted at a later date? If so, how do they proceed?
