Help with small claims form?

Hi

I posted about problems with Jet2 in the consumer forum. It seems like I have no choice but to take them to small claims court in order to get any sort of compensation or refund.

Is there anyone that could help me with the paperwork in the New Year? I have ME which gives me cognition and attention span problems, so I'm really struggling to even read up on what to do!
