Templates?

Hello - I'm looking for a letter template that I can use to make my request official and "proper" so that I won't be fobbed off (I recently received an email referring to it as an informal meeting, which it was not). I've been criticised aggressively and without reason. I would like the manager concerned to provide an explanation of why I was treated the way I was and an explanation of why she said what she said. I can't find anything with a search - can anyone here help?
