S78 request document

Hi there, I have received a letter from Lowell asking to settle a discounted previous credit card debt or they will take me to court. Where can I get a S78 request document to send to them.
Please help.

If the card account was opened in the last 6 years it is likely the agreement will be forthcoming so if there are no other issues/disputes with the account it may be worth looking at the discounted offer and consider negotiating to settle outside of court.
