Income and Expenditure forms

Hello, as a self employed tradesman my money fluctuates and im not too sure on how to fill out the form. Averaging a wage out from last years income doesn't always work, i could be out of work for 2 weeks. I had already said i can pay £40 per month which is within my means and they have asked me to fill the form out. Are there any guidelines to make sure i keep things in my favour so i dont struggle?
