Possible letter template needed

Hi guys I'm in need of a possible letter or template for one etc , my letter writing is terrible ! , I'm enquirering to sainsburys regarding a default I disagree with, I contacted there customer service team and they said they are unable to locate the account on there system so have advised I need to write a letter to lloyds banking group customer services this is where I'm pretty poor to be honest !! The only details I have is the loan ref number , dates of the start of the loan and the date it was defaulted can anybody help or direct me in the right direction please thanks chris
