Equita and their fees

i have just received my statement of account from equita for the last few years and there are anumber of charges which i do ot agree with for example levy fee which it is a total lie and they neveer entered my property.

i need to write a letter to them requesting these charges back. i would be greatfull if someone can help me with the basic letter template refering to the legal side of things with regars to their charges.
