  • Allocation Questionnaire Guide - N149(Small Claims Track)

    Here you will find an online PDF version of the N149 form(Small Claims Track) with form filling facility. -
    Please note that once completed (dependant on your version of PDF reader) the Allocation Questionnaire CAN NOT USUALLY be saved and you are advised to print 3 copies , at least , once completed.

    A guide to completion is as follows however it must be adapted to suit your own circumstances.

    your chosen Court
    County Court

    Claim number and last date for filing with court office these are normally completed by the issuing court , but double check they are completed and the details are correct.

    A SETTLEMENT Tick NO

    B LOCATION OF HEARING Tick NO Unless there is any reason why you feel otherwise.

    C TRACK Tick YES

    D WITNESSES NONE

    E EXPERTS Tick NO To all 4 questions

    F HEARING Tick As applicable
    Include any dates that you may be on holiday , in hospital etc etc
    INTERPRETER Tick as applicable and specify any type of interpreter you may use.


    G OTHER INFORMATION
    Any other information that you consider relevant to the judge to consider your claim to enable him allocate hearing time/resources etc, referring as necessary to any documents you might have attached,whether you feel it should be in small claims track rather than fast tack etc.
